Make a Referral

We try to keep our referral process as simple as possible and generally respond to all submissions within two business days. We also keep our referral form up-to-date with information about when we are at capacity, and when we are able to accept new referrals. 

A few reminders about our services to review before completing a referral:

○ We work with individuals who are 18 or older

○ We are only able to partner with individuals whose support services are paid through a waiver (HSS and VRS are exceptions)

○ We are generally only able to provide services in the Dakota County area (Please feel free to email us at referrals@bbmn.org, if you want to ask about a specific area before submitting a referral. Our service area can change slightly based on our staff)
