Make a Referral

A few reminders about our services to review before completing a referral:

○ We work with individuals who are 18 or older

○ We are only able to partner with individuals whose support services are paid through a waiver (VRS is the only exception)

○ We are generally only able to provide services in the Dakota County area at our twin-cities location (Please feel free to email us at referrals@bbmn.org, if you want to ask about a specific area before submitting a referral. Our service area can change slightly based on our staff)

We try to keep our referral process as simple as possible and generally respond to all submissions within two business days. We also keep our referral form up-to-date with information about when we are at capacity, and when we are able to accept new referrals.
